EXHIBITS REQUIRED TO APPLY FOR A REVERSE EQUITY MORTGAGE
Capital International Financial, Inc., your FHA approved Reverse Equity Mortgage lender, does NOT take into consideration your credit history, NOR require verification of income. Income is not important as in a regular mortgage. We do request, as required by FHA, that you provide us with certain information that verifies who you are and your age. Please be ready when we meet to have the following information ready FROM ALL BORROWERS to complete your Reverse Equity Mortgage application:
In All Cases:
□ A color copy of your valid Florida driver‘s license or Florida State issued picture ID – (face must be recognizable in photo) (obtain from Florida Drivers License Dept. 1-800-617-2000), Must show same address as application. If you do not have one, a copy of your Birth Certificate or a copy of your valid US passport.
□ A copy of your Social Security card, or a copy of your Medicare card. (If you already qualify)
□ A copy of the Warranty Deed by which you took title to the property,
□ A copy of the first 4 pages of the Title Insurance Policy provided you when you purchased.
□ If you have a Mortgage(s): A copy of a Mortgage Company monthly payment statement(s) or coupon(s), showing the account Number and their name and address
1 to 4 Families Residences:
□ Copy of the Declaration (3-4 first) pages of your property and flood insurance policy(ies) and Name and telephone number of the agent(s). ______________________________________________ Tel:_______________

Other Specific Cases (if applicable):
□ Durable Power of Attorney: Please notify us before the Counseling Session. POA must attend counseling. Requirements: Must be Durable (must be valid even if the person becomes incompetent). Must be specific, give the right to encumber the property and include the “Legal Description” of the property. Must be Revocable, and executed by a competent borrower prior to date of application. Please provide a copy of the POA and color copy of the POA’s valid photo ID (photo must be recognizable) and Social Security card at application.
□ Trust: Must be revocable. Please provide copy of the entire document. Consult your attorney.
□ Divorce: please provide copy of divorce decree.
□ Death Certificate: please provide original if deceased owner’s name still on Title.
□ Bankruptcy: Please provide entire package including “Discharge” if you have it. Name and Phone of your attorney. Consult your attorney.
□ Legal Resident(Green Card): legible copy of both faces of the card.
